The following must be completed before a child may attend. 

 

A one-time NON-refundable administration fee of $75.00 per family. A yearly Registration fee of $75.00 due every October. Parents are required to complete an Emergency Information and Immunization Card (Blue Card), provide immunization records and agree to the daycare's Statement of Services, CACFP form, Billing Policy, and Parent Agreement.
